Whats wrong with my gpu ?

It could be a few things. One major thing is your Power Supply is quite small. You may be stressing it and it may have caused damage to your GPU. The GPU itself could be fried because of this. Temp is a bit high as well. Not to bad though. Try a larger PSU and if that does not work. Replace the video card.
